New York-born, California-raised Sanchez gave the tiny Dominican Republic its first medal.
"It was one of the most beautiful races of my life. This is the best possible birthday present," said Sanchez, the first man since Ed Moses to retain the world title.
American Joey Woody clinched the silver with a timing of 48.18 seconds, Greek Periklis Iakovakis (48.24 seconds) won the bronze.
